Units can now use the pdf version of the Officer Report Form for the purpose of
reporting new unit officers.
- Fill out the report
- Print four copies
- Affix the signatures of the unit Commander and Adjutant on all four copies
- Send one signed copy to DAV Auxiliary National Headquarters

- Send one copy to your DAV State Department
